CONTEXT & SCOPE
The record starts with what the facility team needed to decide.
A useful deficiency report should help the facility team locate the issue, understand what was observed, see which test or service task produced the finding, and identify who owns the next decision.
Begin with the exact building, floor, room or system reference. Then separate an observed condition from an interpretation, a proposed correction, and an AHJ requirement. Note whether the item was tested, visually observed, inaccessible, reported by others, or outside the agreed scope. A contractor can explain its service finding and propose work within verified capabilities. The monitoring provider, design professional, owner, fire department, insurer, manufacturer, and AHJ may each have a different responsibility. This resource is general information, not legal or engineering advice.

Record photography should identify useful technical context without exposing private building information.
STRUCTURED HANDOFF
Report Review Checklist
This structure separates building context, installed systems, agreed scope, constraints, open decisions and record handoff. It does not assert universal compliance or an AHJ result.
01
Confirm system, location and asset reference
02
Identify the task and evidence behind the finding
03
Separate observed condition from proposed correction
04
Record inaccessible items and operational limitations
05
Confirm which decision belongs to the facility, contractor, designer or AHJ

YOUR FACILITY
Bring the site, system and current decision.
We will separate routine inspection, an observed deficiency and new project work before proposing the next step.
